Sports Registration should be done through your child's profile in PowerSchool. Click on the "forms" tab on the right hand side fo yoru screen. If your child is not a KMS student, these are the forms you should print off, fill out and return to your child's coach.

KMS Sports Physical Form

This form must be filled out and signed by the students physician prior to participating in a KMS Sport. They are good for 2 years. It is recommended to make a copy of the physical - turn one in to school, keep one copy for your records.

KMS Remote ImPACT Test

Athletes will be required to take an ImPACT Test intheir first season at KMS and again every 2 years until they graduate should they continue with sports in high school. An ImPACT Test is an online cognitive computer test, that will evaluate the brain on a regular “day at rest”, uninjured with no symptoms. This will give us a baseline to work from should an athlete get a concussion. Please take a moment to watch this below video before going into the test itself. I will stress that the student should take this “test” seriously in a calm, quiet environment on a laptop or desktop computer. This is a pass fail/test and has to be passed before participating in the team practice.

We will no longer be using the online Google screening form for winter sports at Kennett Middle School. Instead we will use the same screening strips and system that the school uses each morning.
