KMS Sports Physical Form
SportsPhysicalKMS.pdfThis form must be filled out and signed by the students physician prior to participating in a KMS Sport. They are good for 2 years. It is recommended to make a copy of the physical - turn one in to school, keep one copy for your records.

KMS online ImPACT Test

An ImPACT Test is an online cognitive computer test, that will evaluate the brain on a regular “day at rest”, uninjured with no symptoms. This will give us a baseline to work from should an athlete get a concussion. This is a Pass/Fail test and is important to have done for a safe recovery. It has 3 sections: Name & address, health questions, then a series of "brain teasers" to measure reaction time and challenge the brain. Each athletes will complete a baseline test every 2 years until 12th grade should they continue with sports in hgh school.
Students should take this “test” seriously in a calm, quiet environment on a laptop or desktop computer. No phones or tablets, please. This is a pass fail/test and has to be passed before participating in the team practice.
